On the next page you are asked to provide a few details about yourself. You are then asked to complete a health questionnaire. The information you provide will be kept secure and will only be used by members of the health team providing your care.

Please remember, you are under no obligation to use the Staywell service. However, it is hoped you will do so because it may help to improve the health care provided to you.

Completing the Questionnaire

Although there may seem to be a lot of questions to answer, please don’t be put off. The questions are very simple and take most people around 10-15 minutes to complete.

You can answer yes, no or not sure to each question. Please do not worry if “not sure” is the answer you feel you have to give. It’s not an exam!
